...working in the lab, i was looking at a SCSI HD that was in a small external box. I leaned over it to tak a look at the connections on the power supply... when i leanover, a bit of wire loom touched the power supply, and anoter tester turned the box on... sparks, and 3 holes in my shirt later... it started to work again.

my favourite is when i am working on a printer and it decides to eat the tie my boss makes me wear - nothing better than being sucked closer to a huge chattering dot matrix behemoth while getting rapidly strangled

Backfeed button on dot matrix printer + previous month's carbon copy of all the invoices = office filled with paper. If people didn't insist on sticking soft toys to the equipment with adhesive, I might of actually been able to find the power switch to shut it off.
