We reformat all hard drives prior ro release
in the field and re-install Windows 95 v4.00B.
Hardware is Dell Optiplex PCs GXi and above.
Recently we added digiport cards and receive
the following error on a blue screen when COM3
is initialized (COM3 is attached to a printer):
Blue screen message is:
“An exception 0E has occurred at 0028:C001A256
in VxD VWIN32(01) + 00001262. This was called
from 0028:C027D4F1 in VxD VCOMM(04) + 00000A0D”
The problem seems to occur randomly and only
upon cold boot.
These symptoms do not appear on a PC that has
the original operating system (i.e. not formatted)

Have you tried using a debugger yet and finding out where the instruction generating the exception is called from? You'd be surprised how much light can be shed on a problem in this way (IE called from APM.VXD or something similar)...
