Seen it a number of times over the past month after someone clicked OK.
Safemode and Malwarebytes updated works well

Being a neophyte in many areas, I am going to ask;

Could a nasty person design a malware thing with its' own interface box such that it looked like a Windows box BUT clicking on the little red 'x' actually did something else?

I don't know, so I always (like 3 times in 7 years) use Task Manager and stop the process.

As always, CCT is correct. I have seen different versions of this that are coded so that any click allows it to load. Task manager is the way to go.

Malware Defender 2009 seems to be a rather popular problem these days. Caught it a little while ago, but managed to remove it manually with these instructions (http://www.2-viruses.com/remove-malware-defender-2009).
