Sounds like a memory problem or possibly a motherboard issue. Memory is more likely tho

The first one looks to be a Video Card issue (3dfx16v3.drv) with the VooDoo3 drivers.
The other 2 look towards Memory to me.

My money goes towards the video card on all 3 counts. the first is defiantly video card related, the third is video driver also (been there done that with a xpert2000) and so logic dictates that the second one would be from the video card also.
Try changing the card to something that has a manufacturer that is still in business, or try changing the video drivers to the latest version.

Got to go with the video card or drivers. If your using the latest driver, try using an older driver you know works. I've had a similar problem with an ATI card where the latest drivers really screwed up so I went back to an earlier driver.
If it aint broke, don't fix it....... If it's broke, buy a new one

Gotta disagree with everyone here and say it is the "L" key on the keyboard, it is so blatantly obvious.......
Yeah, I'll put a vote in for video card.

Sounds like it could be a video issue, but it also could be your AGP GART drivers. Make sure you've updated the chipset drivers for your VIA or Intel chipset as well
I've also seen some issues resolved simply by taking the AGP mode in the GART setup from "Turbo mode" to "Standard mode", but of course this is at a performance cost.
Sometimes the most difficult problems have the most obvious solutions - they just get overlooked.
