Buttons and something

Ok this is a small website i need finished. i have a little list each with a subsection. i need the ability to click a button next to a selected area in each subsection. also i want ability to make it so they can put their name in then press submit so its sent to a designated website. how would i do this? im thinking a type of javascript that i can change to add/subtract buttons and sections
