Westell B90-210015-04 DSL Modem

I'm struggling trying to get the Westell B90-210015-04 DSL Modem I bought on line. I got it to recognize the ethernet connection but can seem to get it on line. I'm not a Verizon customer. I presently have a internal modem which is PPOE compliant for my connection but need a external to also connect a laptop.

Can someone giude me thru the setup as I don't have a installation disk. I did download the Diagnostic/installation program that is out there from Verizon. It tried to assign a addresss but said it couldn't find the modem.

Ok, so we need to have some background information...

1. Who is your ISP? do they use PPPoE or a routed bridge configuration?

The default info, after a reset of the modem/router is:
