Osco, that model is the SM56 PCI II modem. <a href="http://e-www.motorola.com/webapp/sps/site/prod_summary.jsp?code=SM56PCI2DF&nodeId=01M0yyK3NVV" target="_blank">The drivers are available here</a> scroll to the bottom of the page and choose your windows version.
Please also note that if this modem was provided in a branded computer, eg Dell, then you should get the drivers from the computer's brand website.
