Unknown NVidia card

Need to find out what type of Nvidia card I have. It's an AGP buss with a fan/heat sink permently glued to the main graphics chip. There is a bar code symbol on the back edge with the characters:
DS/N CN - 01E200 - 44571 - 16R - 00S6 | C/O CN

There are four Samsung 118 RAM chips:
K4D62323HA - QC60

A sticker on the back has the following info in Korean:
1. VGA CARD (180-P0020-0100-E)
2. E-G012-01-1496(B)
3. Nvidia International, Inc.
4. 2001.07
5. Nvidia International, Inc./USA

Any help would be greatly appreciated.
