driver for an OEM WinModem

I hope that someone can supply me with a the file(s)/driver needed to install my model 0637 PCI WinModem. I am running Win98SE. The vendor who supplied it called it a Model 2974, 56kps, v.90, PCI WinModem. The DP/N on the label is 0002513E REV. A00. That's all the information that I have! Please help me if possible; I have downloaded and tried four different files so far from several sites including of course 3Com.
