I have had that happen at least a half dozen times. My dirt cheap company outsources the duplication of our desktop/laptop ghost image CD's to a company that stamps them on really cheap (thin) media. The problem arises as we use the CD's in our Thinkpad and Dell laptops where we have to "snap" the CD in the drive before inserting it. Over time the CD begins to spider crack in the middle. We try to use them as long as possible (to show we are being prudent with company assets), but every once in a while one of the CD's decides to head to the great beyond and take a laptop DVD with it.
