What sort of mouse? ps2, usb, serial, cordless?
Have you checked for spyware, trojans and viruses?

I used to have the same problem on my laptop with my usb mouse when I connected it with the ps2 converter ; this under windows ME ,and happends 3 out of 10 times I rebooted it and I saw the same in my school with w2k advanced server.
While with my laptop I resolve it by not using the ps2 converter ,with the other one I tryed to change the mouse and it didn't work ,then I tried with another OS in the same machine (we have hot swap bays) and the problem disapeard ,so I think I can tell that it's a windows problem .
Sorry I can't help more ....I didn't had the time to investigate it more deep .

that's a point, a true ps2 or an adapter to ps2 from usb or serial?

Is it an optical mouse? If so, try it on a flat surface, with a white piece of paper under it. Do the erratics clear up?

Tried another?
Also in some installations of windows 2000 (on which XP is based) you would get this problem... cure? format and reinstall.
