IT dept year-end evaluation
I need to come up with some sort of year-end survey to give to our users in order for them to evaluate us. I'm not really sure how to organize a questionnaire and ask questions that people will actually answer intelligently
Does anyone know of any tech sites that have good examples of such or have you made one of these yourself before? Any input or help would be appreciated.

The one I came up with from scratch is pretty close to theirs so I guess I'm on the right track.
