I think it's real, but there ARE a couple suspicious parts to it.
1. Guy in white hat has a strange dent in his left leg by his knee.
2. A couple of those guys have some strange white lines by their knees. Seems like an after effect of rubber stamping. But my only question would be why?
3. Large tire closest to left side of image has some mightly clean white ghosting along the tire. At first I thought it was just a clean edge caused by dirt on the tire, but I compared some other edges of the truck and they don't seem to have such a clean contrast to the rest of the image.
4. If you zoom in on different parts of the image, you will notice different image qualities. For example, zoom in on one of the workers. They are heavily bitmapped. The truck is not as badly bitmapped if you zoom in on parts of it. Also there is some bitmapped ghosting around the edges of the workers.
Now that I think about it,.. I am pretty sure it's a fake.

I see what you see about the knees webby...
but the shadows are well done if its a fake...theres no skid marks either or roll marks
the jury is still out here....
heres one the right way up I am looking for a forward view

You need a new option of half real/half fake. I'd say the truck really is overturned, but the workers have been pasted in front of it.

It looks to me that the truck really flipped. Look at the guy with the rag behind the railing, that looks clean. But it seems the guys standing in front were shopped in.

Since imaging is what I do for a living, I thought I'd weigh in on the issue...
This looks real to me. We have ways of determining if pixellation/resolution differences can be found within a single photo, and I couldn't find any. Zooming in on it, the things that look iffy on the guys legs and such, look acceptable when viewed close up. A big issue that usually gives "shopped" images away is shadowing. The shadows under each guy, although small, are similar in angulation, size and brightness, and match the minimal shadowing of the bottom [leading edge] of the truck resting on the dirt. That'd be very difficult to get right. The bent stepup ladder [top right] supports a vehicle rollover idea. [Though, that could've happened any time.]
What doesn't look right? I'm always suspicious of washed out areas...too bright, too white and stuff like that. It's easy to hide shopping in those areas. To me, the tires look flat on top, as if, flipped over, they're still resting on the earth, but 1] who knows what the tires look like on something like this when they're "downside up?", and 2] It's impossible to detect any pixellation difference with all the white surrounding them, especially in a .jpg image.

There may have been roll marks, but not visible in such a small view of the camera, also, there may have been dirt movers along side filling in a pile or something, we can't really draw any conclusion there without knowing that the accident scene was left untouched before this picture was even taken...
