FYI this is usually caused by spyware problem. It causes a problem with the userinit.exe file or it's registry key (HKEY_LOCAL_MACHINE\Software\Microsoft\Windows NT\CurrentVersion\Winlogon
Value: Userinit)

We opened a new branch a few months ago and had a similar problem with at least half of the Dells we installed. It turned out to be the drive imager didn't have the right connector for the drives and the images were corrupt. That was really wierd tho, cuz we ran set up and activated them all just fine. Joined them to the domain and everything.
d) I would choose the format root.
e) Could be the spyware, especially if the user was an admin

I often 'just look around a bit' .. think 'what a mess' .. think fix time vs reformat time ? .. & I recover whatever documents the user might want & just go straight to format, do not pass go, do not collect £200 .. but just do a format & be done !
On the notion of running a repair to fix a corrupted swap file - err wouldn't it be much easier to just delete the pagefile ? A repair option is effectively a re-install anyway, so it might sort out our system files 'for a bit' (probably what it did here) but if you've not got rid of the original infection, you may find yourself going around & around.
