We opened a new branch a few months ago and had a similar problem with at least half of the Dells we installed. It turned out to be the drive imager didn't have the right connector for the drives and the images were corrupt. That was really wierd tho, cuz we ran set up and activated them all just fine. Joined them to the domain and everything.
d) I would choose the format root.
e) Could be the spyware, especially if the user was an admin
Machine part sorted with a repair install but it is rotten with viruses & spyware
